When you refer a patient to us

We make it our business to understand your patient’s needs and we tailor our services to suit them. We aim to make the referral process as simple as possible and to provide you with regular updates via email and SMS.

We know you’re busy so we’ll be in touch as soon as possible with an outcome or a status update. You can download our referral form here or use our handy web form.

Funding eligibility confirmed, client contacted and assessment commenced

We’re pleased to say that they have been approved for the service and we look forward to helping them recover at home.

Summary of services

Your patient has now been confirmed to receive Remedy Healthcare services, a full service summary will be provided via email.

Post service report

Your patient has now completed their Remedy Healthcare program, a full discharge summary will be provided.

As a Patient of Ours

After your referral, your Remedy Care Coordinator will work with you to help you choose the care and support that you need, to ensure your independence and that you feel safe, confident and supported in your own home.

Your Remedy Care Coordinator will contact you to discuss the care and support that you need.

You will receive a confirmation of the services to be provided and ongoing contact from your Care Coordinator while you’re receiving the services, to make sure your care is progressing as planned.

We will liaise with your GP and other healthcare professionals in your team as your needs change.
